Job description

BAYADA Home Health Care is seeking a compassionate and dedicated Full-Time Licensed Practical Nurse (LPN) to join our team in the Norwich, Vermont Hospice office. This office provides hospice care on a per visit basis in homes and facilities across Windsor and Orange counties in VT and Grafton County in NH. A minimum of one year of recent, verifiable clinical experience as an LPN is required.

As a home care nurse, you will be a vital part of a multidisciplinary healthcare team delivering skilled nursing and palliative care, enabling clients to receive necessary medical treatment while remaining at home.

Responsibilities of a Licensed Practical Nurse (LPN):

• Conducting home visits to clients within designated areas.
• Administering medications, wound care, treatments, and procedures as assigned.
• Monitoring client conditions and reporting changes to the Clinical and Client Services Managers.
• Following through on and documenting doctors' orders accurately.
• Performing client assessments as needed.
• Managing case coordination and documentation.
• Recording observations, interventions, and evaluations in electronic medical records using a state-of-the-art tablet.

Qualifications:

• Current LPN license in VT/NH.
• At least one year of recent, verifiable clinical experience as an LPN.
• Graduation from an accredited nursing program, evidenced by transcript or diploma.
• Home care experience is preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Proficiency with computers; prior EMR experience is a plus.
• Ability to travel to assigned cases.
