Responsibilities

The Licensed Practical Nurse (LPN) in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home, within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members. The LPN may not alter the plan of care.

Instructs the patient's family in care and maintenance for the patient and maintaining a healthy environment.

Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and any changes in condition daily.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or reliable public transportation
• CPR Certification
