Job description

We are currently hiring Home Support Workers (HSW) to be part of our team that will deliver highly integrated, community care for Toronto communities in Downtown Toronto and East York. This position requires day shift availability.

Job Description

Being a Home Support Worker is a rewarding career where you will build meaningful relationships with patients and their families, making a real difference in their lives. You will assist patients with personal care and help manage daily activities, enabling them to continue living at home and providing respite for caregivers and family members.

1. Provide basic personal care for medically stable patients under the supervision of a Home Support Supervisor.
2. Assist patients with activities of daily living such as bathing, dressing, toileting, skin care, and socialization.
3. Perform basic meal preparation and light housekeeping duties, including washing dishes, taking out the garbage, doing laundry, and light cleaning within the patient's immediate living area.
4. Provide respite for medically stable patients while primary caregivers are unavailable.
5. Maintain awareness of patient’s abilities, diagnoses, diet, personal interests, and special needs.
6. Travel within the assigned region between patients’ residences or facilities is required.

Qualifications

• Supportive Care Worker Certificate, Health Care Aide Certificate, or completion of Term 1 of a PSW program.
• Valid Vulnerable Sector Police Check and TB Test.
• Current CPR and First Aid Certificates.
• Effective written and verbal English communication skills.
• Ability to interact with others in a caring, tactful, and courteous manner.
