Job summary

Spectrum Health Care is hiring a Home Support Supervisor in Ontario to lead a team of Personal Support Workers in delivering in-home care. The role travels to patients’ homes across York Region and requires active supervision, coaching, and performance management.

The ideal candidate holds current Ontario nursing registration (RPN) and has 3+ years of leadership experience in a community setting, with strong communication skills, a valid driver's license, and proficiency in Word/Excel.

Qualifications

• Current registration with the Colleges of Nurses of Ontario in good standing.
• Minimum three (3) years’ experience with progressive leadership responsibilities as a direct supervisor.
• Exceptional interpersonal skills and ability to handle difficult situations objectively.
• Ability to write reports and communicate effectively in writing and verbally.
• Excellent English verbal and written communication skills.
• Solid knowledge of community nursing and service delivery to patients.
• Valid driver’s license and access to a reliable vehicle.
• Basic proficiency in Microsoft Word and Excel; learning proprietary software.

Responsibilities

• Leads a team of PSWs to provide qualified personal care to patients in a home setting.
• Travel to patients' homes in community and facility settings.
• Collaborate with multidisciplinary teams in community and facility settings.
• Assess patient needs and formulate the plan of care.
• Manage staff, conduct performance appraisals, and address disciplinary action.
• Collect and analyze patient and staff feedback; monitor caseloads, turnover, and performance metrics.
• Address patient concerns and complaints promptly.
• Promote Spectrum Health Care through local committees and community events.
• Engage in ongoing education for personal development.
