Qualifications

• Registered Nurse with active CNO license in good standing.
• Valid driver’s license and access to a vehicle.
• Reliable, flexible, and capable of travel within assigned area.
• On-call coverage required, including after hours.

Responsibilities

• Using assessment tools to determine individualized patient needs.
• Execute and monitor care plans.
• Engage patients through collaborative rapport and trust.
• Maintain and update patient records related to encounters and plans.
