Champlain Rehab Solutions (CRS) is seeking a Registered Dietitian to assess and consult with adults, geriatrics and/or pediatrics in home settings in Kingston, ON. The role involves evidence-based dietetic interventions and coordinating care with the client care team to promote independence.

Qualifications

• Member in good standing with the College of Dietitians of Ontario.
• Minimum 2 years clinical experience.
• Valid Driver's License with access to a vehicle.
• Compassionate and empathetic.
• Self-driven with ability to work independently.
• Skilled in caseload management.
• Skilled at electronic documentation.
• Able to provide virtual care.

Responsibilities

• Provides assessment and consultation for adults, geriatric and/or pediatric clients in their homes.
• Recommends dietetic interventions and coordinates care with the client care team.
• Prepares client reports and plans for clients and caregivers.
• Educates staff on healthy eating and proper nutrition.
• Supports various interventions including diabetes, GI disorders, cancer, and palliative care.
• Promotes safe client care and continuous quality improvement.
