Job summary

ParaMed seeks a Clinical Unregulated Care Provider Supervisor to oversee PSWs and HSWs delivering community-based care. The role ensures adherence to standards, policies, and quality care, with leadership responsibilities and patient safety focus.

The position requires CN/RPN licensure, 5+ years RN/RPN experience, and strong skills in assessment, documentation, and team management. A valid driver’s license and reliable transportation are necessary.

Qualifications

• Current RPN or RN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
• Minimum of 5 years experience as a practicing Registered Nurse or Registered Practical Nurse.
• Comprehensive patient health assessment skills are required.
• Possess an understanding of the principles of community nursing practice.
• Proven experience leading and managing a team.
• Knowledge of clinical documentation standards and practice guidelines is required.
• Excellent organizational, interpersonal, and teaching skills.
• Commitment to professional development in clinical practice that is relevant to home health care.
• Effective critical thinking and problem‑solving skills.
• Must possess a valid driver's license and access to reliable transportation.
• A current Vulnerable Sector Screening (VSS) and C.N.A. Certification as a Community Health Nurse (CCHN(C)) is asset.

Responsibilities

• Supervise and support PSWs and HSWs to ensure practices comply with legislated standards and company policies.
• Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
• Provide ongoing leadership through regular huddles, face-to-face meetings and one-on-ones.
• Initiate patient safety incident investigations with timelines and action plans.
• Support clinical documentation and submission requirements per funder guidelines.
• Engage patients in problem solving, health teaching, and building rapport to identify needs and goals.
• Engage staff in problem solving, offering education, coaching, and mentoring.
• Maintain knowledge and skills to provide safe, efficient, effective service.
