Job summary

ParaMed is seeking a Clinical Supervisor to oversee Health Care Aides delivering in-home care across communities. You will assess, implement, and evaluate clinical interventions to ensure safe, ethical, and high-quality service, while leading the team and ensuring compliance with standards.

You will mentor staff, conduct regular meetings, manage patient safety investigations, and collaborate with nursing to support documentation and funder reporting.

Qualifications

• Licensed Practical Nurse in good standing with CLHA.
• 3–5 years’ nursing experience.
• Valid driver's license and reliable transportation.
• Clear Vulnerable Sector Screening (VSS).
• Experience leading a team.
• Knowledge of clinical documentation standards.

Responsibilities

• Supervise and support HCAs to ensure practices comply with all legislated standards of practice as well as company policies and procedures.
• Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
• Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.
• Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
• Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
• Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.
• Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
• Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.

What You Bring

• Registration as a Licensed Practical Nurse with CLHA in good standing
• Minimum of 3-5 years’ experience as a practicing nurse
• Must possess a valid driver’s license and access to reliable transportation
• Able to provide a current, clear Vulnerable Sector Screening (VSS)
• Comprehensive patient health assessment skills are required
• Possess an understanding of the principles of community nursing practice
• Proven experience leading and managing a team
• Knowledge of clinical documentation standards and practice guidelines is required
• Excellent organizational, interpersonal, and teaching skills
• Commitment to professional development in clinical practice that is relevant to home health care
• Effective critical thinking and problem-solving skills.
