Job summary

Sunnybrook is seeking a Registered Nurse with expertise in home dialysis to assess, plan, implement, advocate, and evaluate care for patients with end-stage kidney disease on home dialysis modalities. You will work as part of an inter‑professional team to provide person‑centred care.

The role involves education for patients and families, home visits, data analysis, and ongoing support as a primary nurse or part of a team. A valid RN license in Ontario and BCLS are required.

Qualifications

• Current Registration with the College of Nurses of Ontario in good standing.
• Current B.C.L.S. Certificate required.
• Baccalaureate degree in Nursing, preferred.
• Certification in Nephrology CNeph(C), preferred.
• Experience with Peritoneal Dialysis and venipuncture preferred.
• Recent (3 year) Hemodialysis experience required.
• Ability to integrate guidelines into policies and procedures.
• Knowledge of MS Office applications.

Responsibilities

• Plan and provide personalized care with each patient.
• Perform and document multiple assessment and follow‑up activities.
• Provide health teaching & family counselling and training related to modality choice and ongoing support as primary nurse or team member.
• Provide education support for home modalities including home visits.
• Analyze clinical data and use critical thinking to determine care/interventions.
• Participate in weekly home dialysis clinics and provide follow‑up care as required.
• Liaise with vendor and community partners.
• Mentor colleagues to improve home dialysis knowledge.
• Participate in inter‑professional rounds, quality initiatives and research.
• Use Nephrology EMR and other applications to document and track patient care.
