Job summary

Ontario Health atHome is seeking a Part-Time B Care Coordinator to assess patient needs, determine service eligibility, and coordinate care plans across home and community settings. You will work with patients, families and a multidisciplinary team to ensure accessible, patient-centered care.

The role requires 2+ years of community health experience, knowledge of the healthcare system and strong communication, problem-solving, and documentation skills; a valid driver’s license and Windows

Qualifications

  • Membership in good standing with Ontario regulatory bodies (e.g., nurses, physiotherapists, occupational therapists, social workers, dietitians, speech-language pathologists).
  • 2+ years of recent experience in community health or a related field.
  • Knowledge of the health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
  • Effective time management, prioritization and organizational skills; ability to work independently and in a busy multidisciplinary environment.
  • Ability to accurately complete required documentation, reports and forms.
  • Valid driver’s license and access to a reliable vehicle.
  • Proficient in a Windows environment.
  • COVID-19 vaccination status submission prior to start date.

Responsibilities

  • Assess care needs with patients and families, determine service eligibility and develop individual care plans.
  • Link patients with service providers and coordinate care plan delivery.
  • Establish supportive relationships with patients and families.
  • Balance patient needs with available resources while respecting values and preferences.
