Job summary

Alberta Health Services seeks a Licensed Practical Nurse (LPN) for Interventionist work in a community home care setting. You will deliver direct nursing care, participate in interprofessional teams, and apply the nursing process in alignment with CLPNA standards.

Your role includes assessing and planning care for clients and supporting families and caregivers. The position is on-site for a 0.80 FTE with days and weekends rotation, requiring a vehicle and compliance with BCLS-HCP and medication

Qualifications

• Completion of an accredited practical nurse education program.
• Active or eligible for registration and a practice permit as a Licensed Practical Nurse with CLHA/CLPNA.
• Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) current within last 12 months.

Responsibilities

• Provide direct nursing care to individuals across the life span.
• Act as part of an interprofessional health care team and use the nursing process.
• Assess, plan, implement and evaluate the needs of Home Care clients and caregivers.
• Collaborate with the Registered Nurse Case Manager to coordinate care.
