Job summary

Saskatchewan Health Authority is seeking a Health Educator for the Red Coat Clinic in Assiniboia to support chronic disease management through education and community engagement. You will work with an interdisciplinary team to empower individuals to achieve health goals and improve self-management.

Responsibilities include training, implementation, evaluation, and delivering group programming in a client-centered approach within the Primary Care Site - Redcoat Clinic Team.

Qualifications

• Bachelor degree in related Health Science or Social Science.
• Valid Class 5 driver's license.

Job description

As a member of the interdisciplinary team and under the directors of the Manager, this position is responsible for the training, implementation, evaluation, and community engagement activities that support chronic disease management initiatives. Working collaboratively with the Primary Health Care team and community partners, the Health Educator assesses client needs and provides education, and group programming. Through a client-centered approach, the Health Educator empowers individuals to achieve their health goals, improve self-management.

Education

• Bachelor Degree in a related Health Science or Social Science discipline

Experience

• Experience or courses in facilitation, conflict and project mgmnt an asset

Knowledge & Abilities

• Excellent organizational skills including the ability to maintain priorities.
• Has demonstrated strong interpersonal and communication skills.
• Knowledge in health needs of individuals living with COPD and chronic conditions
• Knowledge in principles of adult learning
• Requires proficiency with a variety of computer applications
• Ability to lead, mentor, and support staff in a collaborative team environment
• Ability to build and maintain effective relationships/partnerships
• Advanced knowledge of diabetes practices and principles.
• Demonstrates effective verbal and written communication skills.
• Knowledge in chronic disease prevention and management
