THIS POSITION IS REMOTE/WORK FROM HOME IN THE POSTING STATES.

THE POSITION STRONGLY PREFERS A REGISTERED DIETITIAN LICENSURE.

WORK SCHEDULE: Monday - Thursday 8AM - 4:30PM or 8:30AM - 5PM; two evening shifts 10:30AM - 7PM per week, and one shift on Friday per month 9:30AM - 6PM, based on Central Time Zone hours.

Position Purpose

Assists members by completing health assessments and implementing effective solutions and care plans to improve their health and wellness. Works with members at risk or diagnosed with chronic conditions to provide support, encouragement, and education.

Responsibilities

1. Utilize clinical training to support members with chronic health conditions.
2. Perform telephonic assessments to evaluate member needs and clinical risk.
3. Provide telephonic coaching on wellness care plans and programs to improve health and lifestyle.
4. Assist members in developing attainable wellness goals with support and resources.
5. Monitor wellness plans and outcomes, adjusting care as needed to reduce health risks.
6. Engage members in decisions related to maintaining a healthy lifestyle.
7. Coordinate with healthcare providers to align wellness and medical care plans.
8. Maintain accurate member records in compliance with standards.
9. Provide feedback for care and quality improvements.
10. Perform other duties as assigned and ensure policy compliance.

Education and Experience

Requires an Associate's degree and 2-4 years of related experience.

Licenses and Certifications

• Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, Master's in Behavioral Health, or Health Coach Certification.

Pay Range: $26.50 - $47.59 per hour.
