Job summary

SE Health’s Elizz team in Vancouver is hiring part-time Health Care Aides to provide home support to clients in their homes. You’ll assist with daily living, meal prep, light housekeeping, and follow individualized care plans with transfer of function where trained.

Qualifications include CHW/HCA certification with BC Care Aide Registry, current First Aid/CPR, immunization record, and a valid driver’s license. Competitive benefits and training opportunities are offered.

Qualifications

• Availability to work days and/or evenings.
• Community Health Worker (CHW) or Health Care Aide (HCA) certified with BC Care Aide Registry.
• Experience in healthcare.
• Current First Aid (Standard) / CPR Certificate (Level C).
• A valid driver’s license and vehicle.
• Current immunization record including TB test within 12 months.
• Current vulnerable sector check screening.
• Able to work independently as well as part of a team, with excellent organizational skills.
• Well-developed interpersonal and customer service skills, to effectively communicate with clients.
• Effective problem-solving and conflict management skills.
• Basic computer/smartphone skills would be an asset.

Responsibilities

• Follow Care Plans and maintain accurate client records.
• Maintain relationships with clients promoting independence.
• Understand transferable tasks and delegated responsibilities.
• Communicate client condition changes to supervisor and team.
• Identify and promote client safety, seek nurse support when needed.

Position responsibilities:

• Follow Care Plansand maintain accurate client records utilizing the appropriate documentation
• Maintain effective relationships with clients promoting physical and emotional independence
• Clear understanding of the accountability and expectations that are associated with transferable skills/delegated tasks
• Effectively communicate changes in the client condition to your supervisor, and maintain effective relationships with other team members
• Proactively identify, promote and seek to improve client safety as part of your everyday work, and seeks support from the Nurse Supervisor for responding to medical and ethical dilemmas

This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
