Job description

About Ascent Home Care

At Ascent Home Care, we believe exceptional care starts with taking exceptional care of the people who provide it.

We are a nurse-found, nurse-led home care company built by people who understand caregiving from the inside. We've seen what happens when caregivers feel unsupported, schedules become unsustainable, and the people providing care feel like a number. We're building a different kind of home care company. One where caregivers are respected as professionals, supported by accessible leadership, and given the tools they need to provide care they're proud of.

What Makes Ascent Different
You Matter Here

We take the time to know our caregivers, understand their strengths, and support them in doing their best work. Our leadership team stays connected and accessible because we believe good leadership means understanding what's happening in the homes, not only what's happening in the office.

Consistent Client Assignments

Whenever possible, we prioritize consistent client‑caregiver matching so you can get to know the people you support, understand their routines and preferences, and build meaningful relationships with their families.

Consistency is better for our clients, and we believe it's better for caregivers too.

People-Centred Decisions

From scheduling and client matching to communication and care planning, we consider the needs of both our caregivers and the families we serve. We believe high‑quality home care requires listening, communicating clearly, and treating people with respect.

Professional Growth

You’ll have access to ongoing education, clinical guidance, mentorship from experienced nurses, and opportunities to strengthen your skills in areas such as dementia care and complex client needs. We invest in our caregivers because when you feel confident, prepared, and supported, our clients receive better care.

We Are Looking For
Someone Who Cares Deeply

You understand that great care is about more than completing a task list. You notice the little things, treat people with dignity, and understand that kindness, patience, and human connection are an important part of helping someone feel safe and cared for in their own home.

Families depend on you, and you take that responsibility seriously. You show up when you're scheduled, communicate when something changes, and understand that consistency helps build trust with clients, families, and your team.

Growth-Minded

You're open to learning, feedback, and developing your skills. Whether it's strengthening your approach to dementia care, improving communication, or learning new care techniques, you want to continue growing as a caregiver.

Team Player

Home care may happen in a client's home, but you are never working alone. You communicate openly, support your colleagues, ask questions when you need guidance, and work collaboratively with our nursing and leadership team.

What You’ll Do

As a Health Care Aide with Ascent Home Care, you'll provide personalized support that helps clients remain safe, comfortable, and independent in their own homes.

  • Personal care, including bathing, dressing, grooming, toileting, and mobility assistance
  • Medication reminders within your scope of practice
  • Observing and reporting changes in a client's health, behaviour, or condition
  • Meal preparation and assistance with eating when required
  • Light housekeeping related to the client's care and immediate environment
  • Companionship and meaningful social interaction
  • Supporting established routines that promote comfort, dignity, and independence
  • Following individualized care plans and documenting care accurately
  • Communicating clearly with clients, families, and the Ascent nursing team
  • Reporting concerns or changes promptly so our clinical team can respond appropriately
Qualifications
  • Health Care Aide certificate or recognized equivalent
  • Current First Aid and CPR certification
  • Ability to provide a satisfactory criminal record check, including Vulnerable Sector Check
  • Strong verbal and written English communication skills
  • Professional, dependable, and comfortable working independently in clients' homes
  • Ability to follow care plans, document accurately, and communicate changes in client condition
  • Valid driver's licence and access to a reliable vehicle are considered assets
  • Additional languages are considered an asset
What We Offer
  • Competitive pay: $19.00 to $21.00 per hour
  • Health benefits: Traditional benefits for eligible employees
  • Flexible Spending Account: Additional flexibility to support your individual health and wellness needs
  • Paid mileage: Reimbursement for eligible travel between client visits
  • Employee referral bonus: Earn a bonus when you refer a great caregiver who joins our team and meets program requirements
  • Consistent client assignments: Whenever possible, we match you with the same clients so you can build trust and meaningful relationships
  • Flexible scheduling: Full‑time, part‑time, and casual opportunities based on client needs and your availability
  • Ongoing training and clinical support: Guidance from our nursing team, including dementia care education
  • Professional growth: Opportunities to strengthen your skills and grow with Ascent
  • Accessible leadership: A leadership team that listens, communicates, and stays connected to our caregivers
