Job summary

Ascent Home Care is seeking Health Care Aides to provide personalized in-home care, promoting safety, comfort, and independence for clients. You will perform personal care, meal prep, light housekeeping, and companionship, while following individualized care plans and documenting changes, under the guidance of experienced nurses.

• Must have strong verbal and written English communication

Responsibilities

• Provide personalized support to help clients stay safe, comfortable, and independent at home
• Perform personal care activities (bathing, dressing, grooming, mobility) as per care plan
• Give medication reminders within scope of practice and report health changes
• Assist with meal prep and eating, light housekeeping
• Provide companionship and meaningful social interaction
• Follow care plans and document care accurately
• Communicate clearly with clients, families and nursing team
• Report concerns promptly to clinical team

What You'll Do

As a Health Care Aide with Ascent Home Care, you'll provide personalized support that helps clients remain safe, comfortable, and independent in their own homes.

Depending on the client's care plan, your responsibilities may include:

• Personal care, including bathing, dressing, grooming, toileting, and mobility assistance
• Medication reminders within your scope of practice
• Observing and reporting changes in a client's health, behaviour, or condition
• Meal preparation and assistance with eating when required
• Light housekeeping related to the client's care and immediate environment
• Companionship and meaningful social interaction
• Supporting established routines that promote comfort, dignity, and independence
• Following individualized care plans and documenting care accurately
• Communicating clearly with clients, families, and the Ascent nursing team
• Reporting concerns or changes promptly so our clinical team can respond appropriately

Qualifications

• Health Care Aide certificate or recognized equivalent
• Current First Aid and CPR certification
• Ability to provide a satisfactory criminal record check, including Vulnerable Sector Check
• Strong verbal and written English communication skills
• Professional, dependable, and comfortable working independently in clients' homes
• Ability to follow care plans, document accurately, and communicate changes in client condition
• Valid driver's licence and access to a reliable vehicle are considered assets
• Additional languages are considered an asset
