Job summary

Providence Health Care is seeking Licensed Practical Nurses to join Crosstown Clinic in Vancouver. You will provide harm reduction nursing care, administer medications, and ensure coordinated, compassionate treatment for clients with substance dependence.

The role emphasizes assessment, education, advocacy, and collaboration with a multidisciplinary team, supporting patients toward stability and access to care.

Qualifications

• Graduation from a recognized Practical Nurse program.
• Registration with the BC College of Nurses and Midwives as an LPN.
• Current basic cardiac life support certification.

Responsibilities

• Provide specialized harm reduction nursing care and administer medications per established protocols.
• Monitor clients' health, respond to clinical needs, and ensure safety.
• Participate in assessments, care planning and evaluation for individual needs.
• Connect clients with community and primary care supports as needed.
• Educate clients, advocate for dignity and access to care.
• Collaborate with an interdisciplinary team including physicians and social workers.
• Support orientation of new staff and students in clinical areas.
• Contribute to care conferences and safe clinical environment.
• Assist with discharge planning based on patient status and supports.
