Job summary

M’Chigeeng First Nation is seeking a Gichi-Aya’aa (Elders) Community Navigator to plan and deliver educational and social activities for Elders at the Wellness Centre. The role requires coordinating resources, maintaining equipment, and integrating Anishnabek cultural values in all program activities.

Two regular full-time positions with benefits involve liaison with the Home Care Program and other departments. A valid driver’s license and VSS clearance are required.

Qualifications

  • OSSD with relevant post-secondary courses and minimum two years’ work-related experience.
  • Health Care Aide Certificate and/or PSW Certificate required.
  • Vulnerable Sector Screening (VSS) current within 30 days, original copy or certified.

Responsibilities

  • Plan, organize and implement educational and social activities for Elders.
  • Identify resources to meet identified needs for Elders.
  • Incorporate Anishnabek cultural values and Anishinabemowin into delivery.
  • Inform community members of upcoming events and activities.
  • Provide administrative reports for funding and accountability.
  • Oversee equipment loan program and ensure maintenance and reporting.

Gichi-Aya’aa (Elders) Community Navigator

Two Positions Available

Department: Health Services – Home & Community Care Program

Classification: Community Services

Classification Level: CS-3

Employment Status: Regular Full-time with benefits

Reporting Relationship: The Gichi-Aya’aa (Elders) Community Navigator will report directly to the Home Care Program Manager of M’Chigeeng Health Services; maintain communication within the department and internal working relationships with all Departments within M’Chigeeng First Nation.

Essential Functions
  • Plan, organize and implement educational and social activities for the elderly members of our community.
  • Formulate concrete goals and objectives and identify appropriate internal and external resources to meet identified needs in the provision of support services to Elders of the community.
  • Incorporate and promote Anishnabek cultural values, beliefs, practices and Anishinabemowin into program delivery.
  • Ensure community members are informed of upcoming events and activities organized by the Home Care & Community Program.
  • Provide administrative reports in a timely manner, as required for funding and accountability.
  • Provide administrative duties in relation to the medical equipment loan program and ensure all equipment is maintained. Notify Program Manager of any equipment deficiencies.
  • Ensure Home & Community Care Program passenger bus is maintained according to Ministry of Transportation Standards.
  • Provide care and home support services as required for elderly residents and community band members.
  • Practice strict confidentiality, privacy, ethical and non-bias approaches.
  • Engage in regular liaison with the Home & Community Care Team, other related programs and/or departments within the Community and external agencies.
  • Participate in on-going professional development as it relates to the position.
  • Coordinate and participate in cultural events and ensure cultural components are incorporated and practiced in all areas of the Elders Program where possible.
  • Input support services into the electronic medical record management software system on a regular and consistent basis.
  • Will take an active role and participate in interdepartmental strategic planning activities by assisting in the formulation of annual program goals and objectives for the Home & Community Care Program.
Qualifications & Requirements
Minimum
  • OSSD with relevant Post Secondary level courses with a minimum of two (2) years’ work related experience
  • Health Care Aide Certificate
  • Personal Support Worker Certificate
  • Work experience with volunteers, developmentally challenged and the elderly.
Preferred

College Diploma in health care and/or social services related program with three (3) years adequate related work experience

  • Excellent change management and relationship-building (partnering & networking) skills
  • Well organized with ability to multi-task, work well under pressure, and to prioritize work
  • Valid Class “G” Drivers License
  • Knowledge of First Nation issues, culture, traditions, customs and practices
  • Fluency in Anishinabemowin or a willingness to learn
  • Vulnerable Sector Screening (VSS) required (Current 30 days, original or certified copy)
