Job summary

Chartwell Retirement Residences in Victoria seeks an LPN to support residents' well-being through quality care in line with policies.

You will perform basic assessments, administer medications, document care plans, and collaborate with Health Care Aides and the management team.

Qualifications

• Graduate from a recognized LPN program with valid registration
• Knowledge of geriatric care principles and regulatory acts
• Strong communication and patient-centered care skills

Responsibilities

• Provides basic assessments and treatment, personal care, medication administration and delegated health services to residents, and supports Health Care Aides
• Communicates with residents and families to ensure needs are met and services are satisfactory
• Administers medications within LPN scope and follows regulations
• Completes routine assessments and monitors resident health status
• Documents services and maintains accurate charting
• Monitors and records residents' physical and mental condition
