You will conduct basic assessments, administer medications, and support Health Care Aides under established policies and care plans.

Qualifications

• Graduate from a recognized LPN program with valid registration.
• Knowledge of geriatric care, rehabilitation, and nursing principles.
• Ability to communicate with residents and families with empathy.

Responsibilities

• Provides basic assessments, personal care, medication administration, and delegated health services.
• Communicates with residents and families to ensure needs are met and service quality is high.
• Documents services and charts with accuracy according to policies and procedures.
• Ensures care plan services are delivered at approved levels and monitors resident health status.
• Performs delegated nursing skills and reports observations to the Assisted Living Manager.
