Job summary

Vancouver Coastal Health in North Vancouver, BC is seeking a Community Care Assistant to provide home support services within the Home Support team. Under supervision, you will help clients with daily living activities, observe changes, and report to the care team to promote independence and safe care at home.

You will collaborate with caregivers and health professionals, utilize a client care information system, and follow harm-reduction and culturally safe practices while prioritizing

Qualifications

• Graduation from a recognized Health Care Assistant Program.
• Registration with BC Community Health Worker and Care Aide Registry.
• Experience with dementia/geriatric/palliative populations considered an asset.
• Valid BC driver's license and access to a vehicle; local travel may be required.

Responsibilities

• Provide home support services to clients under supervision.
• Assist with ADLs and IADLs to promote independence.
• Collaborate with health care team and caregivers.
• Document significant changes and report to supervisor.
