Job summary

Vancouver Island Health Authority is seeking a Community Health Worker to provide personal care support in clients' homes and help maximize independence. You will follow established care plans and assist with daily living activities in a compassionate, patient-centered manner.

Travel between multiple sites is a regular requirement; a valid BC Driver's License is needed. The role starts Oct 2, 2026, with a 1.00 FTE schedule and shift from 1:30 PM to 10:00 PM.

Qualifications

• Home Support/Resident Care Attendant/Health Care Assistant Certificate or equivalent.
• Food Safe Certificate is required.
• Valid BC Driver's Licence and active registry in BC Care Aide & CHW Registry.

Responsibilities

• Provide personal care assistance and support to clients and caregivers to maximize independence at home.
• Assist with activities of daily living and follow care plans as directed.
• Travel between multiple sites and use a personal vehicle if required.
