Qualifications

• Experience interacting with persons with dementia, geriatric and/or palliative population an asset.
• Must have Valid BC Driver's license and access to a vehicle; local travel may be required.

Responsibilities

• Provide home support services to clients within client-centered care framework.
• Assist clients with ADLs/IADLs and report significant changes to the health care team.
• Collaborate with health care team and caregivers to maximize independence.
• Motivate and involve clients in their care and contribute to care planning.
