Brant Community Healthcare System in Brantford, Ontario, is seeking an experienced Geriatrics Emergency nurse to provide expert clinical assessment and care in the ED. You will work with GEM nurses and liaise with community services to improve elderly care across the region.

Ideal candidates will have 5+ years in geriatrics or emergency settings, CN/O registration, and BCLS certification; a Gerontological Nursing Certificate is preferred.

Qualifications

• Baccalaureate Degree in Nursing from an accredited program; Master’s preferred.
• Current registration with the College of Nurses of Ontario (CNO) required.
• Current Basic Cardiac Life Support (BCLS) certification.
• Minimum of five years in geriatrics and/or emergency services.
• Gerontological Nursing Certificate preferred or mandatory.

Responsibilities

• Perform comprehensive assessments of elderly ED patients to guide disposition and care.
• Act as resource/coach to ED staff on geriatric care.
• Participate in ED quality improvement initiatives and program evaluation.
• Coordinate with GEM and community resources to link seniors to services.
• Educate staff and promote best practices in elder care within the ED.

Position Summary

Reporting to the Clinical Manager of Geriatrics, your goal in this position is to be responsible for providing expert clinical assessment and for making recommendations regarding the health care needs of the geriatric population in our Emergency Department (ED). You will work collaboratively with other Geriatric Emergency Management (GEM) nurses across the province, as well as liaising with other hospitals, the community and retirement home services in our region to build the province’s capacity to deliver more effective emergency services for our geriatric population. You are able to work independently and collaboratively with all members of the health-care team including patients and families. You have a strong knowledge of community resources and can apply principles of patient and family centered care. You demonstrate exceptional skills in performing assessments, health promotion and health teaching for the older adult population. You use critical thinking, problem-solving, and decision-making to provide the most appropriate safety plans for your patient. You have excellent communication and interpersonal skills. You demonstrate an ongoing commitment to professional development. You can work in a fast-paced environment and effectively prioritize patient care needs. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.

Primary Responsibilities

• Complete comprehensive assessments and identify the needs of elderly patients in the ED, with the goal of avoiding unnecessary hospitalizations and to safely discharge patients to the community.
• Act as a resource/coach to ED staff in the care and management of elderly patients with complex medical and social issues.
• Participate in program evaluation and ongoing ED quality improvement initiatives.
• Work collaboratively with ED dementia team to facilitate timely and safe discharge of elderly patients from the ED and to link them to community resources including Specialized Geriatric services (SGS).
• Work collaboratively with in-patient geriatrics team as required to ensure comprehensive care to identified high risk geriatric patients.
• Communicate assessment findings and needs of admitted patients to members of the receiving unit’s interdisciplinary team.
• Initiate and maintain links with other GEM Nurses in the HNHBB Region to advance clinical outcomes for older adults receiving care in the ED.
• Analyze and interpret best evidence related to care of older adults.
• Design and provide educational and learning opportunities for staff and acts as a resource for related issues and problem solving.
• Promote a model of best practice in Elder Care nursing and an elder friendly philosophy of care within the ED.
• Provide best practice education to nurses in the ED and in other clinical areas to enhance the quality of care for the elderly.
• Build relationships with the local retirement homes within the community to build capacity and collaboration with the goal being ED avoidance.

Mandatory Qualifications

• Baccalaureate Degree in Nursing from an accredited nursing program, Master’s Degree in Nursing is preferred.
• Current registration with the College of Nurses of Ontario (CNO) required
• Current Basic Cardiac Life Support certification (BCLS)
• Minimum of five (5) years of demonstrated current expertise in geriatrics and/or emergency services
• Gerontological Nursing Certificate preferred, but mandatory completion is required
• Knowledge of principles of adult education, preferred
• Member of Registered Nurses Association of Ontario, preferred
