Job summary

Extendicare is seeking a Registered Dietitian to lead resident nutrition/hydration care. You will assess residents, develop and adjust nutrition plans, and collaborate with the interdisciplinary team to support optimal care at our Fort Macleod location.

You will provide counseling, monitor at-risk residents, and contribute to menu reviews, nutrition education, and quality improvement initiatives in a long-term care setting. Part-time position with competitive benefits.

Qualifications

• University degree in area of Food and Nutrition from an approved university
• 1-2 years of experience in gerontology, rehabilitation, community/complex/long term care an asset
• Current registration in the applicable province's regulatory College of Dietitians
• Membership in Dietitians of Canada and the Gerontology Network
• Must be knowledgeable of Provincial Food Premises Regulation, CCHSA Accreditation • Canada, Provincial/Health Authority regulatory requirements, and Residents' Bill of Rights
• Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills
• Must have proficient computer skills using Microsoft Office applications and Synergy (Microsoft Office Suite, Word, Excel, PowerPoint, and Access), Point Click Care/POC/eMAR
• Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one

Responsibilities

• Assess nutrition status for residents on admission, annually, and with significant status changes
• Determine nutrition and hydration risk levels to prescribe interventions and follow-up care
• Develop nutrition plans of care for newly admitted residents
• Evaluate and modify nutrition plans for residents under RD
• Document resident nutrition care and provide counseling/education
• Monitor high/medium risk residents in RD oversight
• Participate in interdisciplinary resident care teams and committees
• Coordinate continuous quality improvement of nutrition and hydration care and dining services
• Review and approve seasonal menus and therapeutic textures
• Oversee Restorative Feeding, Dining and Enteral Nutrition programs
• Collaborate with Administration on resident nutrition needs
