Job summary

CarePartners is seeking Registered Nurses (RN) and Registered Practical Nurses (RPN) to provide flexible home health nursing across Ontario, including in-home, school, clinic, and retirement-home settings. You will join a supportive team focused on quality patient care and meaningful community impact.

You will develop and maintain Care Plans, deliver nursing services both in-person and virtually, and collaborate with patients, families, and the interdisciplinary team in various care environments.

Qualifications

• RN or RPN degree/diploma required.
• Active CNO registration in good standing.
• Excellent communication and interpersonal skills.

Responsibilities

• Develop and maintain an effective care plan.
• Provide nursing services in-person and virtually.
• Deliver care in-home, clinic, retirement home or school.
• Collaborate with patients, families and care team.
