Lumenus Community Services is seeking a Family Home Visitor to provide parenting support and community resources to families with young children (prenatal to age six) who are at risk due to social isolation, economic insecurity, or language barriers. The role is part-time fixed term (22.5 hours/week) through Sept 1, 2027, with visits delivered in clients' homes or in the community in Toronto.

  • College Diploma in Early Childhood Education and registration in good standing with the College of Early Childhood Educators, or a high school diploma with relevant experience.
  • 3–5 years’ experience working with children and families in community settings.
  • Experience or familiarity with families new to Canada is preferred.

Responsibilities

  • Plan home visits to support parenting goals and child development.
  • Administer developmental screens and document visits.
  • Connect families with community resources and parenting programs.
  • Develop and deliver parent education and child programs.
  • Collaborate with Toronto Public Health Nurses and staff.
  • Maintain records for family files.

POSITION TYPE: Part-Time Fixed Term (22.5 hours/week) - Fixed Term until September 01, 2027

POSTING CLOSING DATE: September 15, 2026

SALARY RANGE: $30,000 - $32,000

Lumenus Community Services is looking for a Family Home Visitor, responsible for providing parenting support and community support and resource information to families with young children (prenatal to age six) who are “at risk” on the basis of social isolation (i.e. new to the country and away from family and friends), economic insecurity, language barriers, etc. The service is delivered in the client’s home or community.
The Family Home Visitor may work in partnership with Toronto Public Health Nurses and/or other community and Lumenus Community Services staff members to ensure that families are supported in assisting their children to grow to their optimal potential.

KEY RESPONSIBILITIES:
  • Assist families to identify realistic goals and expectations regarding parenting and child growth and development
  • Plan home visits to meet goals/objectives
  • Assist families to identify and build on the strengths of their children
  • Administer the Nipissing District Developmental Screens (N.D.D.S.)
  • Model positive parent-child interaction
  • Provide parenting information specific to healthy growth and development e.g. breastfeeding, nutrition, stimulating play activities, home safety etc.
  • Assist families to link with parenting programs and other community resources as needed
  • Document all family visits and maintain all records and forms required for family files in a timely fashion
  • Support the development and delivery of child and parenting programs and services
  • Develop Parent Education and Parent/Child Programs
  • Assist with set-up, clean-up, and general health and safety of the environment
  • Assist with program outreach and promotion
  • Assist with program registration and evaluation
  • Develop and maintain positive working relationships with program staff and program participants
QUALIFICATIONS/EXPERIENCE:
  • College Diploma in Early Childhood Education and, registration in good standing with the College of Early Childhood Educators is a must, or a High School Diploma coupled with relevant experience
  • 3 -5 years’ experience working with children and their families in community settings
  • Familiarity with the challenges of families who are new to Canada
  • Personal parenting experience is an asset

SKILLS/KNOWLEDGE:

  • Spoken and written fluency in English
  • Spoken and written fluency in Farsi/Dari
  • Ability to communicate effectively with adults and children from diverse ethno-cultural backgrounds
  • Ability to listen and empathize with a non-judgmental approach
  • Ability to understand and implement the “family-centred” model of service provision to families
  • Ability to effectively partner with the community (i.e. Public Health Nurses)
  • Knowledge and understanding of growth and development of young children (from birth to 6 years of age)
  • Strong problem-solving skills in responding to the needs of families
  • Ability to plan activities in response to goals set by partner agency (TPH) in collaboration with the family. Identify resources needed to meet weekly objectives, review and modify as required
  • Ability to advocate for the needs of families
  • Report writing and computer skills (Microsoft Word and e‑mail)
  • Ability to travel to various locations within Toronto
  • Access to reliable transportation to travel throughout Toronto regions on a regular basis
  • Strong time management and organizational skills
  • Demonstrated ability to be flexible
  • A personal cell phone in good working condition is required for this job to be used in the case of emergencies
