POSTING # 25-05-09POSITION TYPE TERM (6 Months), FULL-TIMEBackground: The Ma Mawi Wi Chi Itata Centre is currently seeking an individual for the position of Care Planner. The incumbent will assist in assuming responsibility for providing guidance to the HBCI Mentors and establishment of family care plans. The Family Care Planner will ensure access to services that are community, strength, and value-based and are aimed at stabilizing and supporting the youth/families in crisis as a part of the family's care plan.Brief overview of some duties:
- Act on behalf of the Team Lead in the Team Leads absence.
- Assign all referrals to appropriate mentor.
- Assist in creation of goal based care plans for referred families.
- Meet and collaborate with the HBCIS Mentor regarding the pre-planned activity schedule/template, and overall care plan.
- Maintain weekly records, reports, incident reporting, communication logs and statistics detailing all relevant activities with respect to achievement of program goals.
- Ensure receipts, mileage etc. forms legible and organized.
- Initiate and participate in care plan meetings, consultations, and reviews.
- Provide effective interpersonal, verbal, and written communication, demonstrating effective and timely documentation, within timeline guidelines.
- Attend orientation, staff meetings, training sessions, and/or supervision meetings.
- In-home or phone follow-ups as per the Family Care Plan.
- Develop/update and maintain a Community Resource Listing/binder that is culturally and community-relevant.
- Facilitate ongoing, effective communication networks between families, youth, children, community sister agencies/partnerships.
- Report any serious/relevant incidents to the HBCI Team Lead.
- Ensure compliance with all standards, regulations, policies, and legislative requirements.
- Other duties as assigned by Home Based Crisis Intervention Team Lead, Executive Team.
Qualifications:- Youth Care Diploma or acceptable equivalent and related experience.
- Care planning and healing plan development and implementation with the participants.
- Working knowledge of crisis intervention strategies.
- Demonstrated experience developing and maintaining an effective working relationship with Indigenous participants.
- Non-Judgmental problem-solving ability, conflict resolution, and decision-making.
- Needs to have the ability to work cooperatively with others and contribute as a team member.
- Ability to work under stressful situations.
- Strong oral and written communication skills.
- Understanding of Indigenous values.
- Must have Valid Class 5 Driver’s Licence.
- Must provide current (within the last three months) Criminal Record Check with Vulnerable Sector Query, Child Abuse Registry Check and be subject to Prior Contact Check.
