Job summary

Family- in Pembroke, Ontario is seeking Direct Support Providers to help individuals achieve ISP goals and participate in community life. DSPs promote independence, safety, and social inclusion while supporting rights and responsibilities.

Responsibilities include assisting with activities, attending appointments, documenting progress, and following plans under supervision; shifts may include evenings or weekends as needed.

Qualifications

• High School diploma required; experience with adults with Developmental Disabilities is preferred.
• Ability to follow directions and work independently in the community.
• Excellent communication and problem-solving skills; bilingualism is an asset.

Responsibilities

• Be familiar with ISP goals and assigned tasks to meet these goals.
• Support individuals to access community activities and safe living.
• Attend meetings and appointments; report back to Case Manager.
• Document support provided and activities; collect data as directed.
• Provide transportation for activities and meetings; complete time cards.
