Wage Range: $39.17 to $48.96 per hour (To commensurate with experience and consistent with UHN compensation policy)
Hours: 37.5 Hours per Week

Status: Temporary Full-Time

Position Summary

As an integral member of Allied Health, the Discharge Coordinator is accountable to the Program Managers, ensuring discharge related timelines are established, transparent and followed throughout the patient stay in collaboration with interprofessional team.

Duties

  • Communicate effectively with internal and external stakeholders and sources, ensuring excellent customer service regarding discharge planning (i.e. interprofessional team, Ontario Health at Home, other facilities)
  • Assist with discharge planning, and provide the patient with a discharge and patient care conference letter.
  • Communicate and update changes in discharge dates to Program Assistant, Service Coordinator, Ontario Health at Home and reissue updated Discharge Letters to patients after consultation with key core team members.
  • Track discharge timelines to ensure certain time-sensitive activities are completed in time for planned discharge date (e.g. Ontario Health at Home referrals, and transitional facility referrals)
  • Single point of contact and collaboration for Unit Charge Nurse, Unit Manager, care team, Ontario Health at Home and patient when planning for complex and/or Alternate Level of Care risk patients.
  • Coordinate and chair biweekly ALC rounds, identifying and problem-solving barriers.
  • Manage and ensure patients designated ALC are informed, and ensure ALC and WTIS orders are completed.
  • Attend internal and external information sessions regarding resources that could support discharge planning.
  • Attend daily safety huddles, daily flow huddles, weekly rounds for three inpatient units, and manage changes to rounds report.
  • Coordinate and execute any repatriation agreements as directed by Unit Manager, and when all other options have been exhausted.
  • Demonstrate flexibility in dealing with competing priorities on a daily basis.
  • Advocate to internal and external providers on behalf of the patient to access resources that would support direct discharge to the community.
  • Participate in pre-admission patient meetings to highlight considerations that will support discharge planning back to the community.
  • Single point of contact for Unit Manager and team to guide, support, and communicate with ALC escalated patients.
Qualifications
  • Bachelor’s degree with a clinical background, i.e. health-related field, required.
  • Member in good standing with respective college.
  • Experience in a healthcare environment required.
  • Experience in coordinating discharges and/or patient flow preferred.
  • Knowledge of standards of practice and applicable health care legislation.
  • Knowledge of resources available for patients returning to the community.
  • Ability to network and nurture relationships within the healthcare industry.
  • Knowledge of quality/performance improvement theory and practice required.
  • Leadership abilities required.
  • Excellent computer skills required (Word and Excel).
  • Excellent verbal and written communication skills required.
  • Excellent interpersonal skills.
  • Self-directed individual with the ability to work both independently and in a group setting.
  • Must be proficient in English.
  • Excellent attendance required.
