Job summary

South Riverdale Community Health Centre seeks a visionary Director, Integrated Primary Care to lead the delivery of comprehensive primary care programs in East Toronto, ensuring equitable access and high-quality services.

The role requires strategic leadership over interdisciplinary teams, accountability for performance, and collaboration with system partners to advance community health priorities and address social determinants of health.

Qualifications

• Masters degree or equivalent with management experience in primary care or public health.
• Strong leadership experience guiding interdisciplinary teams.
• Proven ability to drive quality improvement and accountability.
• Commitment to health equity, anti-oppression, and community engagement.
• Experience with budget planning and resource stewardship.

Responsibilities

• Lead delivery and performance of integrated primary care programs.
• Strengthen existing services and develop new models of care.
• Ensure services reflect best practices and equity-informed approaches.
• Oversee quality improvement and performance monitoring to hit targets.
• Represent SRCHC in system planning and cross-sector partnerships.
• Lead HR activities including recruitment, onboarding, and development.
• Manage program budgets and strategic allocation of resources.
• Foster a learning culture and positive team environment.
• Plan and implement collaborative programs across two Ontario Health Teams.
