Job summary

South Riverdale Community Health Centre (SRCHC) invites applications for a Nurse Practitioner – Primary CONNECT. This full-time/part-time role is based onsite in Toronto and focuses on health promotion, education and disease prevention within a collaborative IPCT setting.

The position requires RN(EC) registered nursing with Ontario College of Nurses credentials, extensive primary care experience, and strong communication skills to support high-need populations.

Qualifications

  • Graduate degree in nursing from a recognized university.
  • Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario.
  • 3-5 years’ experience in a community setting or related nursing experience.
  • Proficiency with health history taking, assessments and nursing management planning.

Responsibilities

  • Direct Client Care – conducts comprehensive health assessments across the lifespan.
  • Manages episodic and stable chronic conditions in primary care with physician consultation as needed.
  • Provides telephone screening, health advice and referrals.
  • Performs performance of prescriptions and orders laboratory tests within scope.
  • Assists with prenatal care, home visits, and coordination of referrals.
  • Participates in case conferences to coordinate services for clients.
