Job description

ConnectWell Community Health is seeking a Senior Manager, Primary Care to provide leadership across our Community Health Centre sites in Renfrew County. Reporting to the Director of Community Health Centres, this role is ideal for an experienced healthcare leader who is passionate about interprofessional care, people leadership, and equitable health outcomes in community-based settings. The work location will be Beachburg ON, with the need to travel to sites in Renfrew, Eganville, and Cobden, and some flexibility to work remotely.

What You’ll Do
  • Lead interprofessional primary care teams across multiple sites.
  • Advance evidence-based care pathways and consistent clinical practices.
  • Coach and support team members; build strong, engaged teams.
  • Lead quality improvement, risk management, and regulatory compliance.
  • Champion health equity, trauma-informed, and culturally safe care.
  • Build partnerships with community providers and health system partners.
  • Support clinical program budgets, staffing models, and resource planning.
What You Bring
  • Master’s degree in Nursing, Public Health, Health Administration, or related field (or equivalent experience).
  • 8+ years of leadership experience in community-based or primary care environments.
  • 3+ years leading clinical teams in interprofessional models of care.
  • Strong knowledge of Ontario’s health system, quality improvement, and health equity.
  • Registration with a relevant Ontario regulatory body (e.g., RN, NP, SW, PT) is an asset.
