Job summary

Community Living Hamilton is seeking a Direct Support Facilitator for Supported Independent Living to help clients live independently and reach personal goals. You will advocate for clients and coordinate services in the community, focusing on daily living, budgeting, and housing supports.

Position Summary

The Direct Support Facilitator – Supported Independent Living is responsible for assisting clients living in the community to sustain their independence and improve skills of daily living. The incumbent Helps Build Great Lives by advocating our clients’ interests and well-being with community partners and assisting them in reaching their goals.

Responsibilities
  • Assume responsibility for the annual Individual Support Plan ( ISP)for assigned clients.
  • Develop individualized PDPs that reflect each client’s wishes, abilities, personal and family relationships, spirituality, and culture.
  • Collect and maintain required consent forms, medical documentation, and other relevant records.
  • Assist clients in developing annual goals related to skill development, recreation, independent living, employment, health and wellness, housing, education, general interests, and interpersonal skills.
  • Coordinate client intake into and discharge from the Supported Independent Living (SIL) program.
  • Meet with assigned clients monthly to assess needs and identify opportunities to strengthen independence through advocacy, coaching, and skill development.
  • Attend medical appointments when staff participation may support improved health outcomes.
  • Provide logistical support, including coordinating transportation, meetings, and appointments.
  • Provide developmental services support to clients involved with the justice system or other legal matters, when required.
  • Support clients in obtaining housing and completing rental agreements and related documentation.
  • Promote a client-centred approach that encourages collaborative decision-making.
  • Know, respect, and advocate for the rights of clients.
  • Build on clients’ strengths to encourage skill development, self-determination, independence, and community inclusion.
  • Develop and maintain professional rapport with clients based on respect, dignity, inclusion, and appropriate personal boundaries.
  • Coach clients in activities of daily living, including housekeeping, meal preparation, social skills, and responsible daily habits.
  • Assist clients with annual budgeting, taxes, and other financial matters.
  • Support clients in independently planning meals while providing guidance related to nutrition, budgeting, dietary restrictions, and personal preferences.
  • Gather, maintain, and appropriately share information with stakeholders using written and verbal communication.
  • Seek relevant information to better understand program activities and clarify expectations.
  • Provide appropriate and constructive feedback to clients, supervisors, co-workers, and external parties.
  • Demonstrate professional conduct in the community that positively represents clients and Community Living Hamilton.
  • Ensure documentation and reporting meet regulatory requirements and internal policies and are completed using professional, accurate, and objective language.
  • Treat colleagues with respect, kindness, and understanding.
  • Contribute to a welcoming and professional workplace that recognizes the skills, experience, and contributions of team members.
  • Participate regularly in team meetings and review relevant meeting documentation.
  • Share information with team members and other teams to promote learning and consistency of care.
  • Promote the safety, security, and wellness of clients, co-workers, and visitors.
  • Work in compliance with applicable legislation, organizational policies, and regulatory requirements.
  • Identify and contribute to opportunities for continuous improvement.
  • Participate in required training and ongoing professional development.
  • Promote and demonstrate the Vision and Mission of Community Living Hamilton in the workplace and broader community.
  • Work in accordance with the Occupational Health and Safety Act and Regulations and Community Living Hamilton’s Health and Safety Policy.
  • Report workplace hazards and contribute to ongoing workplace safety improvements.

Qualifications
  • Post-secondary education as a Developmental Service Worker, Social Services Worker or equivalent.
  • Prior experience working in the developmental services field, ideally working with adults in a supported independent living setting.
  • Certified in First Aid certificate
  • Knowledge of, and commitment to, the philosophy of inclusion
  • Knowledge of Services and Supports to Promote the Social Inclusion of Persons with
  • Developmental Disabilities Act, 2008.
  • Thorough knowledge of developmental disabilities and Behaviour management.
  • Excellent observation skills and written and verbal communication skills
  • A valid driver’s license, clean driver’s abstract and access to a reliable vehicle to facilitate travel to various sites
  • Proven ability to demonstrate cultural competency with the ability to address service needs in a diverse client group
  • Fluency in a language other than English preferred
  • Experience with diverse client groups preferred (age, culture, program type, etc.)
  • Additional training in mental health/dual diagnosis preferred
  • Additional formal training in additional topics relevant to the position strongly preferred
