Job summary

Community Living Hamilton is seeking 2 Full-Time and 3 Part-Time Direct Support Facilitators to join our Intensive Support Residential Program, supporting individuals with developmental disabilities, mental health challenges, addictions, and homelessness.

You will provide goal-oriented support, promote independence, and work with multidisciplinary teams across shifts. A DSW/SSW/CYW or related diploma is preferred, with first aid, CPR, and a valid G license.

Qualifications

• DSW, SSW, CYW, or related Social Services diploma/degree.
• Two years' experience supporting individuals with developmental disabilities, mental health, addictions, homelessness, or complex support needs preferred.
• Ability to build effective professional relationships based on respect, dignity, inclusion, and healthy boundaries.
• Experience developing and implementing Person Directed Plans and Individual Support Plans.
• Strong communication, problem-solving, and interpersonal skills.
• Commitment to person-centred supports and community inclusion.
• Valid First Aid and CPR Certification.
• A valid G class driver’s license with a clean 3-year driver’s abstract, and proof of insurance in good standing.

Community Living Hamilton supports more than 1,600 adults and children with diverse abilities to live meaningful, inclusive lives in their communities. We are currently seeking 2 Full-Time and 3 Part-Time Direct Support Facilitators to join our Intensive Support Residential Program supporting individuals with developmental disabilities, mental health challenges, addictions, and experiences of homelessness.

This is an existing vacancy.

Pay Rate: $29.28 per hour

Schedule: This position follows a rotating schedule that includes days, evenings, weekdays, and weekends. Day Shift (7 am to 3 pm); Evening shift (3pm – 11 pm); Night shift (11 pm -7 am). Employees will participate in an alternating day and evening schedule, including every other weekend, based on operational requirements and posted schedules.
