Job summary

Sunny Field Residential Homes LLC in Arizona is hiring full-time Direct Care Worker (DCW) / Direct Support Professional (DSP) to provide direct care for individuals with developmental disabilities, adults and children. Shift is Mon-Fri 2pm–10pm; must be 21+ and have a valid driver's license.

Position involves monitoring health and safety, administering medications, meals, transporting to appointments, implementing HAB objectives, documenting duties, and helping with calendars and menus.

Qualifications

• Clear criminal and central registry background check required.
• Must be 18+ with a valid driver’s license (if applicable).
• Proficient in written and spoken English.

Responsibilities

• Monitor health and safety of individuals in the home.
• Administer medications as prescribed by a medical professional.
• Prepare meals (breakfast, lunch, dinner, snacks).
• Transport individuals to appointments, visits, and activities.
• Implement HAB objectives/goals per ISP team.
• Implement behavior plans when applicable.
• Engage with individuals actively during shifts.
• Complete required documentation before leaving shift.
• Assist with monthly activity calendars and weekly menus.
• Ensure the home is clean and in good repair.
• Other duties as assigned.

Reports to: Group Home Manager

Place of Duty: Group Homes

Job purpose: To plan and carry out direct care of individuals with Developmental Disabilities Adult/Children.

Key Responsibilities:

• Monitor and ensure the health and safety of all individuals living in the home.


• Administer medications as prescribed by a medical professional.


• Prepare all necessary meals (breakfast, lunch, dinner, snacks etc.).


• Transport individual to appointments, visits, and activities/community events.


• Implement HAB objectives/goals as agreed upon by the ISP team.


• When applicable, implement behavior building plans.


• Actively interact with individuals in the home throughout your shift.


• Complete all required Documentation before leaving shift.


• Assist with preparing monthly activity calendar and weekly menus.


• Ensure the home is clean and in good repair.


• Other Duties as assigned.

Qualifications:

• Clear criminal and central registry background check (Required)


• At least 18 years of age


• Valid drivers license with clean driving record (If applicable)


• Employee also agrees to maintain all required DDD certifications and a valid fingerprint card throughout the duration of their employment with Sunny Field Residential Homes LLC.


• Proficient with the English language: written and verbal

Education and Experience:

High School Diploma, and minimum one year experience in the DDD or Behavioral Health Field. Must apply for a Fingerprint Clearance Card within 7 days of hire.

Must have Article 9, Prevention and Support, CPR and First Aid certifications within 90 days of hire.

Compensation: $15.50 - $16.00 per hour
