Job summary

Sunny Field Residential Homes LLC in Arizona is hiring a Direct Care Worker (DCW) / Direct Support Professional (DSP) for full-time overnight shifts 10pm–8am. You will monitor health and safety, administer medications, prepare meals, transport residents, and maintain documentation.

Requirements include 18+ years old, a clean background, valid driver's license if applicable, and English proficiency. Certifications (Article 9, Prevention & Support, CPR/First Aid) within 90 days and fingerprint

Qualifications

• Clear criminal and central registry background check (Required).
• At least 18 years of age.
• Valid driver's license with clean driving record (If applicable).
• Maintain DDD certifications and a valid fingerprint card during employment.
• Proficient with the English language: written and verbal.

Responsibilities

• Monitor and ensure health and safety of individuals in the home.
• Administer medications as prescribed.
• Prepare meals for residents (breakfast, lunch, dinner, snacks).
• Transport individuals to appointments, visits and activities.
• Implement HAB objectives/goals with ISP team.
• Implement behavior building plans when applicable.
• Interact with individuals actively during shifts.
• Complete required documentation before leaving shift.
• Assist with monthly activity calendar and weekly menus.
• Keep the home clean and in good repair.
• Perform other duties as assigned.

Available Shifts

Full-time M-T Overnight Shift 10p-8a

(We are also looking for PRN's As needed)

Reports to: Group Home Manager

Place of Duty: Group Homes Job purpose: To plan and carry out direct care of individuals with Developmental Disabilities Adult/Children.

Key Responsibilities

• Monitor and ensure the health and safety of all individuals living in the home.
• Administer medications as prescribed by a medical professional.
• Prepare all necessary meals (breakfast, lunch, dinner, snacks etc.).
• Transport individual to appointments, visits, and activities/community events.
• Implement HAB objectives/goals as agreed upon by the ISP team.
• When applicable implement behavior building plans.
• Actively interact with individuals in the home throughout your shift.
• Complete all required Documentation before leaving shift.
• Assist with preparing monthly activity calendar and weekly menus.
• Ensure the home is clean and in good repair.
• Other Duties as assigned.

Education and Experience

High School Diploma, and minimum one year experience in the DDD or Behavioral Health Field.

Must apply for a Fingerprint Clearance Card within 7 days of hire.

Must have Article 9, Prevention and Support, CPR and First Aid certificates within 90 days of hire.
