Job summary

VHA Home HealthCare in Ontario is seeking a Dietitian to assess and provide consultation to adults, geriatric and pediatric clients in their homes, ensuring coordinated care with the client care team. You will develop dietetic interventions, educate staff and families, and promote independence through tailored nutrition plans.

Qualifications include registration with the College of Dietitians of Ontario, at least 2 years of clinical experience, and ability to work independently, drive to client

Qualifications

  • Member in good standing with the College of Dietitians of Ontario.
  • Minimum 2 years clinical experience (optimal).
  • Valid Driver’s License with access to a vehicle.
  • Compassionate and empathetic.
  • Self-driven with ability to work independently.
  • Able to provide virtual care.

Responsibilities

  • Prepares client reports for health professionals to analyze for treatment.
  • Prepares plans and presentations for clients and their caregivers to provide thorough information on their diet and goals.
  • Educates internal and external staff members about healthy eating and proper nutrition.
  • Supports the following areas of intervention: failure to thrive, dysphasia, feeding problems, enteral feeding, diabetes, cancer, GI disorders, weight management, and palliative care.
  • Helps patients coming to terms with their illness and advises about special and time-specific diets.
  • Supports safe client care through the development of knowledge and skills in accordance with continuous quality improvement.

Pay Range

Estimated annual earning potential: $80,000–$130,000.

Actual earnings may vary based on availability and service volumes

VHA's Dietitian is responsible for assessing and providing consultation, recommendations of dietetic interventions to adults, geriatric and/or pediatric group of clients in their homes using evidence-based best practice. They will ensure coordinated delivery of care through ongoing communication with the client care team regarding plans and schedules for interventions, problem solve in collaboration with the client and care team to create opportunities for more independence and promote VHA’s core beliefs and values.

Key Areas of Accountability

VHA's Dietitians have the following key areas of accountability, but not limited to:

  • Prepares client reports for health professionals to analyze for treatment.
  • Prepares plans and presentations for clients and their caregivers to provide thorough information on their diet as well as their goals.
