Job summary

VHA Home HealthCare (VHA) is seeking an Occupational Therapist to join our team serving pediatric clients in homes and schools in the York Region. You will assess, consult, and implement interventions to promote independence and quality of life using evidence-based practice.

You will collaborate with the client care team to plan schedules, coordinate services, and document progress. Onboarding and professional development support are provided within a flexible, inclusive environment.

Qualifications

  • Degree in Occupational Therapy from a recognized school.
  • Member in good standing with the College of Occupational Therapists of Ontario.
  • Valid Driver's License with access to a vehicle.
  • Compassionate and empathetic.
  • Self-driven with ability to work independently.
  • Skilled in caseload management.
  • Skilled at electronic documentation.
  • Able to provide virtual care.
  • A registered ADP authorizer (asset).

Responsibilities

  • Assess and implement occupational therapy interventions per professional guidelines.
  • Implement client interventions based on assessments and care plans.
  • Ensure services are evidence-based and evaluated with outcomes.
  • Coordinate care through ongoing communication with the client care team.
  • Document assessment findings, treatment plans, progress and discharge summaries.

Occupational Therapist - York Region (Adult) (Healthcare)

Pay Range

Estimated annual earning potential:$80,000–$130,000.

Actual earnings may vary based on availability and service volumes

As an Occupational Therapist at VHA, you will be assessing and providing consultation, recommendations and interventions to a diverse group of pediatric clients in their homes and/or schools using evidence-based best practice. You will ensure coordinated delivery of care through ongoing communication with the client care team regarding plans and schedules for interventions, problem solve in collaboration with the client and care team to create opportunities for more independence and promote VHA's core beliefs and values. #INVHARH #LI-JO1

Key Areas of Accountability
  • Assesses and implements occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.

  • Implement client interventions based on assessment information, client input and in conjunction with the client's integrated care plan.

  • Ensure that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.

  • Ensure coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.

  • Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required b the profession, referral source and the organization.

  • Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.

Required Qualifications:
  • Degree in Occupational Therapy from a recognized school of Occupational Therapy.

  • Member in good standing with the College of Occupational Therapists of Ontario.

  • Valid Driver's License with access to a vehicle
