Job summary

VHA Home HealthCare in Ontario is seeking a licensed Occupational Therapist to assess and treat clients in home, school, and community settings. You will develop individualized intervention plans and document progress to support independence and quality of life.

Collaborate with the care team to coordinate services, ensure best practice, and use outcome measures to guide care. Flexible scheduling and a strong commitment to client-centered care are core to this role.

Qualifications

• OT degree and licensure in Ontario
• Active registration with College of Occupational Therapists of Ontario
• Valid Driver's license and access to a vehicle
• Ability to work independently in home/community settings
• Experience with caseload management and electronic documentation
• Capacity to provide virtual care as needed

Responsibilities

• Assess and implement occupational therapy interventions per guidelines and standards.
• Implement client interventions based on assessment info and integrated care plan.
• Ensure services are evidence-based and monitored with outcome measures.
• Coordinate care through ongoing communication with client’s team.
• Document assessment findings, plans, progress, and discharge summaries.
• Support safe client care with continuous quality improvement.

Pay Range

Estimated annual earning potential: $80,000-$130,000. Actual earnings may vary based on availability and service volumes.

VHA's Occupational Therapist have the following key areas of accountability, but not limited to

• Assesses and implements occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.
• Implement client interventions based on assessment information, client input and in conjunction with the client’s integrated care plan.
• Ensure that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
• Ensure coordinated delivery of care through ongoing communication with all members of the client’s team regarding plans and schedules for interventions.
• Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required b the profession, referral source and the organization.
• Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.

Required Qualifications

• Degree in Occupational Therapy from a recognized school of Occupational Therapy.
• Member in good standing with the College of Occupational Therapists of Ontario.
• Valid Driver's License with access to a vehicle
• Compassionate and empathetic
• Self-driven with ability to work independently
• Skilled in caseload management
• Skilled at electronic documentation
• Able to provide virtual care
• A registered ADP authorizer (asset)
