Job summary

Southbridge Care Homes in Stratford, Ontario is seeking a Registered Dietitian to join our dynamic Food and Nutrition Department. You will ensure residents receive nutritious, delicious meals prepared and served with love.

This part‑time opportunity features daytime hours (8am–4pm) at Hillside Manor LTC, with 17.5 hours bi‑weekly. You will conduct nutritional assessments, develop care plans, and collaborate with the Food Service Manager and care team.

Qualifications

• Registered Dietitian eligible for College of Dietitians of Ontario.
• Geriatric nutrition knowledge required.
• 3–5 years in long-term care preferred.
• Proficient with computer applications and MS Office.
• Familiarity with RAI MDS 2.0 and PointClickCare is an asset.

Responsibilities

• Consult with the Food Service Manager and Director of Care on nutrition and hydration needs.
• Perform nutritional assessments using appropriate tools.
• Develop and monitor individualized diet plans and care goals.
• Coordinate with the care team to implement nutrition interventions.
• Maintain communication about changing resident needs with the Food Service Manager.
• Review seasonal menus for nutritional adequacy and provide approval.
• Educate residents, families, and team members on nutrition and hydration.

Salary Range: $42.00 per hour

Status: Part-Time, Vacancy

Responsibilities

• As a member of the Clinical Team, the Dietitian consults with and advises the Food Service Manager and Director of Care concerning nutrition and hydration program needs. The Dietitian is responsible for nutritional assessment, nutritional care planning, implementation, and follow-up for residents.
• Completes nutritional assessments for each resident using RAI MDS 2.0 as required.
• Determine an individualized resident’s diet and other nutritional interventions to meet his or her needs by involving the resident, family members and care team.
• Develop, implement and evaluate nutritional care plans that include identification of nutritional concerns, goals and interventions for all residents.
• Maintain excellent communication with the Food Service Manager regarding the changing needs of the residents.
• Review the two seasonal menus for nutritional adequacy and provides approval in writing.
• Educate residents, family members, and team members regarding nutrition and hydration recommendations.
• Act as a member of the Interdisciplinary Team, acts as a representative for Southbridge, through participation in regional and professional activities as appropriate.
• Participate in interdisciplinary committees related to resident care, facility operation and Quality indicators as required.

Qualifications

• Registered Dietitian, eligible for membership with the College of Dietitians of Ontario.
• Extensive knowledge of geriatric nutrition
• 3-5 years’ experience in long-term care setting preferred.
• Proficiency with computer applications, Microsoft Office, and the internet. Experience with RAI MDS 2.0 desirable, familiarity with software PointClickCare an asset.
• Excellent organizational, time management, and multi-tasking skills.
• Knowledge of Occupational Health & Safety practices, principles, and legislation.
• Excellent oral and written communication skills are essential.
• Customer-focused attitude, with demonstrated ability to build and maintain effective team relationships with staff and residents.
• A negative vulnerable sector check completed within 6 months of hire date.
• Completion of a 2 Step TB Skin test within the last 12 months.
