Job summary

Southbridge Care Homes in Ontario is seeking a Registered Dietitian to join a dynamic nutrition department across two long-term care homes: Southbridge Lakehead and Southbridge Roseview. You will ensure nutritious meals are prepared and served with care, and you will support residents' dietary needs.

Qualifications

• Registered Dietitian eligible for membership with the College of Dietitians of Ontario.
• Extensive knowledge of geriatric nutrition.
• 3–5 years' experience in long-term care setting preferred.
• Proficiency with computer applications, MS Office; RAI MDS 2.0 desirable; familiarity with PointClickCare an asset.
• Excellent organizational, time management, and multi-tasking skills.
• Knowledge of OH&S practices and legislation.
• Excellent oral and written communication skills; customer-focused attitude.

Responsibilities

• Consult with the Food Service Manager and Director of Care on nutrition and hydration needs.
• Complete nutritional assessments for residents using RAI MDS 2.0.
• Determine individualized diets with input from residents, family and care team.
• Develop, implement and evaluate nutritional care plans with goals and interventions.
• Maintain communication with the Food Service Manager regarding resident needs.
• Review seasonal menus for nutritional adequacy and approve in writing.
• Educate residents, families, and team members on nutrition and hydration.
• Participate in interdisciplinary teams and regional professional activities.
• Join committees related to resident care, facility operation and quality indicators.

Southbridge, the owner and operator of several long-term care homes and retirement communities in Ontario, is currently seeking a Registered Dietitian to become part of a dynamic food and Nutrition Department. You will ensure that our residents are provided with nutritious and delicious meals, that are prepared and served with love. You will also be supporting two homes which makes this opportunity full-time.

RESPONSIBILITIES:

• As a member of the Clinical Team, the Dietitian consults with and advises the Food Service Manager and Director of Care concerning nutrition and hydration program needs. The Dietitian is responsible for nutritional assessment, nutritional care planning, implementation, and follow-up for residents.
• Completes nutritional assessments for each resident using RAI MDS 2.0 as required.
• Determine an individualized resident’s diet and other nutritional interventions to meet his or her needs by involving the resident, family members and care team.
• Develop, implement and evaluate nutritional care plans that include identification of nutritional concerns, goals and interventions for all residents.
• Maintain excellent communication with the Food Service Manager regarding the changing needs of the residents.
• Review the two seasonal menus for nutritional adequacy and provides approval in writing.
• Educate residents, family members, and team members regarding nutrition and hydration recommendations.
• Act as a member of the Interdisciplinary Team, acts as a representative for Southbridge, through participation in regional and professional activities as appropriate.
• Participate in interdisciplinary committees related to resident care, facility operation and Quality indicators as required.

QUALIFICATIONS:

• Registered Dietitian, eligible for membership with the College of Dietitians of Ontario.
• Extensive knowledge of geriatric nutrition
• 3-5 years' experience in long-term care setting preferred.
• Proficiency with computer applications, Microsoft Office, and the internet. Experience with RAI MDS 2.0 desirable, familiarity with software PointClickCare an asset.
• Excellent organizational, time management, and multi-tasking skills.
• Knowledge of Occupational Health & Safety practices, principles, and legislation.
• Excellent oral and written communication skills are essential.
• Customer-focused attitude, with demonstrated ability to build and maintain effective team relationships with staff and residents.
• A negative vulnerable sector check completed within 6 months of hire date.
• Completion of a 2 Step TB Skin test within the last 12 months.
