Job summary

Black Health and Social Services Hub in Brampton is hiring a Registered Dietitian to deliver diabetes education and management support for adults and seniors with Type 2 diabetes. You will work within an interdisciplinary team to provide counseling, group education, and outreach at community locations.

The role involves chart reviews, documentation, and collaboration with partners to improve BAC community health outcomes in Peel Region.

Qualifications

• Accredited dietetics education and registration with the College of Dietitians of Ontario.
• Community-focused internship and hands-on diabetes care experience.
• Active CDE status or progress toward it is preferred.

Responsibilities

• Provide nutrition assessments for pre-diabetes and Type 2 diabetes clients.
• Develop individualized nutrition and diabetes-management plans with the care team.
• Document program activities and medical records accurately and timely.
• Lead diabetes education sessions and coordinate with inter-disciplinary team.
• Maintain client records in EMR with privacy and consent compliance.
