Job summary

Illustr is seeking a Registered Dietitian (RD) to provide community-based nutritional care for diabetes prevention and management.

The RD will participate in developing, implementing, monitoring and evaluating diabetes programs for individuals, families and the community, in collaboration with the RN CDE and other providers.

You will promote partnerships and act as an ambassador for Napanee Area Community Health Centre across South East Ontario, coordinating community events and services.

Qualifications

• The Registered Dietitian (RD) provides community-based nutritional care functions in the prevention and management of diabetes.

Responsibilities

• They participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community affected by diabetes.
• The RD works closely with the Registered Nurse CDE and others to achieve a successful regional Diabetes Education Program.
• The RD acts as an ambassador for the Napanee Area Community Health Centre in the community and promotes positive partnerships with other providers to maximize the capacity for diabetes interventions in South East Ontario.
