Job summary

The Health Centre, serving the Kitchener-Waterloo region, is recruiting a Registered Nurse for the Diabetes Education Program. The RN provides community-based health assessment, diabetes self-management education, insulin starts and adjustments, and behaviour change support for people living with type 2 diabetes in the region.

Working within a client-centered health team, the RN collaborates with CHKW staff, Primary Care Practitioners, Endocrinologists and community partners to develop, deliver

Qualifications

• BScN or equivalent combination of education and experience.
• Current registration and good standing with the College of Nurses of Ontario.
• Three years nursing experience, preferably in a community setting.
• Experience with the development and delivery of health education and promotion programs.
• Strong health assessment and clinical skills.
• Experience in diabetes education and care, preferably with Certified Diabetes Educator qualifications.
• Proficiency with Microsoft Office and EMR systems.

Responsibilities

• Provide individual and group education, health counselling and behaviour change support within RN scope of practice.
• Implement client-centred insulin initiation and/or ongoing diabetes management orders as authorized by referring/primary care practitioner.
• Maintain accurate clinical notes in accordance with guidelines and CHKW policy.
• Participate in development, delivery and evaluation of Diabetes Education programs.
• Ensure privacy and confidentiality and maintain client safety.
