Job summary

Sunnybrook is seeking a Nurse Practitioner to lead the Rapid Diabetes Assessment and Referral (RADAR) Clinic within a collaborative endocrinology and diabetes team. You will develop specialized expertise in diabetes care while coordinating care across inpatient, outpatient, and emergency pathways.

You will mentor others, contribute to evolving diabetes care models, and provide patient-centered education, insulin management, and technology-enabled treatment planning.

Qualifications

• Current RN (EC) registration with the College of Nurses of Ontario in good standing.
• Master's degree and Nurse Practitioner certification.
• Current BCLS.
• Strong clinical assessment and communication skills.
• Ability to work autonomously within an interprofessional team.

Responsibilities

• Assessing patients with diabetes in an outpatient setting.
• Developing and adjusting individualized treatment plans.
• Managing diabetes medications and insulin therapy, including pump initiation and adjustment.
• Providing longitudinal follow-up and monitoring treatment response.
• Collaborating with Endocrinology and other services for coordinated care.
• Refining workflows to support patient-centred care.
• Participating in quality improvement, program development, and education.
• Contributing to innovative diabetes care within the Integrated Community Program.
