Job summary

Qualifications

• Minimum 2 years post-secondary education
• Social Service Worker Diploma, Community Support Worker Diploma, or Human Services Diploma
• Minimum 2 years experience working with adults with intellectual disabilities and/or co-occurring mental health disorders; or
• Equivalent combination of education and experience may be considered

Responsibilities

• Assisting residents in maintaining safe and healthy lifestyles, and monitoring for changes in health, appearance, behavior and activity patterns.
• Identifying a crisis, defuse the situation, determine an intervention strategy, and contact necessary supports.
• Helping residents make healthy choices and follow pre-established medical orders.
• Planning and supporting residents in daily activities including chores, recreation, learning, physical fitness, and money management.
• Monitoring and maintaining a clean, safe, and secure environment for residents.
• Assisting residents with personal hygiene skills and tasks.
• Completing required training within Momentum and Regional Health Authority timeframes and meeting competency levels.

The DSW is responsible for:

• Assistingresidents in maintaining safe and healthy lifestyles, and monitoring forchanges in their general health, appearance, behavior, and patterns ofactivities

• Completingrequired, on-going training within Momentum and the Regional Health Authorityrequired timeframes and meeting required competency levels.

Minimum requirements:

• Minimum2 years post-secondary education or
