Why this role is important?

Basic Function

Working in a hybrid setting on the Specialty Health Network (SHN) Support Program, the Field Clinical Nurse Liaison will be a primary point of contact to the assigned primary health care provider(s) (HCP). In addition, the Clinical Nurse Liaison is responsible for coordinating patients’ access to their prescribed therapy, and ensuring flawless customer service for the patients and HCPs. The Clinical Nurse Liaison will deliver thorough expert communication and problem-solving skills to provide patients with available resources, and quality outcomes related to their therapy.

Nature and Scope

The Clinical Nurse Liaison will maintain a patient case load and will be providing care, not limited to disease state education, molecule education and training to patients and caregiver(s), medication reimbursement coordination, service coordination, in-clinic and telephone support, monitoring of adherence, where required, and side effect management. Building strong working relationships, collaboration, effective communication, and attention to detail both internally and externally are key responsibilities to meet specific team and business objectives. In addition, the Clinical Nurse Liaison will attend pre-booked internal and external training sessions, complete self-learning assignments and independent studies while adhering to Specialty Health Network Standard Operating Procedures, Policies, and the College of Nurses Standards of Practice.

Principal Accountabilities

  • Responsible for assigned HCP case load related to navigating and coordinating care requirements for their patients, including assisting the patient with access to medication
  • Act as the primary resource & point of contact for patients, physicians, health care professionals and other key stakeholders.
  • Coordinates & facilitates the patient care plan from enrollment and throughout therapy
  • Provide status updates thorough documentation in patient file to ensure continuity of care is maintained and that any changes in patient care are clearly identified and transitioned accordingly
  • Provide in-clinic training & build relationships on behalf of the program
  • Conduct individual and group patient education sessions based on specific needs.
  • Administer medications as required in a clinical setting and perform minor procedures under the direction of the physician in accordance with provincial regulatory requirements.
  • Manage and schedule patient appointments to support care plan adherence as required.
  • Address disease state and product-related inquiries.
  • Work collaboratively with the pharmaceutical company representatives
  • Respect confidentiality, advocate for patients/families to facilitate positive outcomes, and ensure that ethical issues are identified and resolved according to the Canadian Nurses Association Code of Ethics
  • Demonstrate commitment to Continuous Quality Improvement
  • Maintain high standards of professional practice by adhering to the ethics of the profession and keeping abreast of the latest professional advances
  • Assume responsibility for personal and professional development
  • Navigation of pharmacy dispensing and care coordination
  • Other responsibilities as required by Specialty Health Network

Qualifications, Specific Skills and/or Competencies:

  • Licensed Registered Nurse with a current, unrestricted status in the province of employment. BScN preferred, however, a combination of experience and education will be considered
  • Five or more years direct clinical experience in Dermatology, including experience with biologic specialty medications biologic experience and chronic disease management is an asset, specialty biologic experience is strongly preferred
  • CPR certification required
  • Confident in delivering health education, including but not limited to patient self-injection training.
  • Ability to work autonomously with little supervision
  • Knowledge of case management processes, recent experience with Patient Support Programs (PSP) preferred
  • Demonstrated customer/account focus and consistently deliver on the SHN Support Program value proposition
  • Fluently bilingual in both written and spoken languages is an asset (French & English)
  • Strong telephony skills
  • Strong organizational and problem-solving skills with attention to detail
  • Ability to provide an organized, efficient approach to case management
  • Possession of excellent skills in assessment, communication, critical thinking, and decision making
  • Ability to demonstrate understanding of Specialty Health Network policies, legal considerations, and professional ethics
  • Proficiency of computer skills not limited to : Microsoft applications and web based applications (EMR)
  • Ability to adapt to a changing environment
  • Commitment to the principle of quality client-centered service delivery
  • Knowledge of theory and implementation of Continuous Quality Improvement processes
  • Must be able to establish and maintain a secure home office environment in accordance with company security policies and procedures
  • Holding a valid driver’s license and access to an insured personal vehicle

Education (check education required for position)

This position requires a University degree

Discipline: BScN preferred

This position requires a designation in good standing with an unrestricted license to practice.
