The Day Treatment program is an in-person and virtual SUD treatment program for individuals with mild to moderate addictions. The goal is to provide evidence and strength based, trauma informed care short-term care to support individuals on their recovery journey. This position will provide case management, facilitate groups, and provide supportive counselling.
This position is part of a larger Mental Health and Additions team. This case management position provides assessment, care coordination, group work, and ongoing case management services.
The role involves engaging individuals with psychosocial, mental health, and addiction needs; providing SUD treatment facilitating access to treatment and community resources; supporting harm reduction and recovery-oriented goals; collaborating with interdisciplinary teams; and assisting with crisis intervention, discharge planning, and ensuring continuity of care. The position emphasizes person-centered, trauma-informed, and evidence-based practices to improve health outcomes and reduce barriers to care.
