Job summary

Weeneebayko Area Health Authority (WAHA) is seeking a qualified Mental Health Worker - Addictions to support adults dealing with addictions in Moosonee, ON. The role emphasizes culturally informed care within Indigenous contexts and requires collaboration with a multidisciplinary team.

The successful candidate will provide case management, counselling, and linkage to community resources, ensuring timely access to services and adherence to care plans in a remote northern setting.

Qualifications

• Diploma in Social Service Work or related field required.
• Bachelor’s degree in social work or related field preferred.
• Registered with the Ontario College of Social Workers and Social Service Workers.

Responsibilities

• Provide mental health and addictions support to clients.
• Ongoing case management for adults living with addictions that may or may not be seeking treatment.
• Case management can include the provision of psychiatric consultation services.
• Liaise with the circle of care to ensure ongoing case management and ongoing relapse prevention for clients.
• Seek supervision and any impasse or areas out of scope of practice as needed.
• Coordinate timely access to mental health, developmental and/or addictions services for adults with complex needs.
• Develop and implement individualized plans of care with short- and long-term objectives and follow up services.
• Monitor and evaluate the service delivery and counseling progress of adults accepted for service.
• Refer and link clients and caregivers to a range of community resources (housing, legal supports, income supports, treatment, health supports, meals, etc.).

Who You Are

• Significant experience working with Indigenous and First Nations People
• Superior verbal and written communication skills
• Demonstrated case management, counselling, and referral experience working within mental health and/or addictions recovery settings
• Knowledge of and sensitivity to issues of gender, race, sexual orientation, and issues impacting diverse communities
• Understanding of the Mental Health Act and related legislation

What You’ll Do

• Monitor and evaluate the service delivery and counseling progress of adults accepted for service
• Refer and link clients and caregivers to a range of community resources [including housing, legal supports, income supports, treatment, health supports, meals, etc.]

What You Bring

• Bachelor’s degree in social work of related field preferred
• Registered with the Ontario College of Social Workers and Social Service Workers

• Ability to communicate in the local native Cree language is an asset
